Payments are due at the beginning of each session, via check or cash.
Insurance
I am in-network with Premera and First Choice Health.
I am an out-of-network provider with all other insurance companies. Many insurance companies will reimburse clients for a percentage of out-of-network mental health costs, so I encourage my clients to research this option. I can provide you with a monthly invoice to submit to your insurance provider for reimbursement. Please verify your mental health benefits beforehand.

Before you arrive for your first session, please fill out the Client Intake Form, and read and sign the Disclosure Statement and the Notice of Privacy Practices & Statement of Client's Rights.
